Used 5.9 Cummins (1994-2007): what to check before you buy
The 10 documented failures of this engine, laid out in the order you inspect a truck: paperwork and VIN, a cold start, a code scan, under the hood, the test drive and a look after it. Each line is the sign as our record describes it, linked to the full failure page. At the end: what each repair costs.

1Before you go
Run the VIN and ask for the paperwork on the campaigns below: a done recall or program is a solved problem, an open one is a free repair the seller has not had done.
- Run the VIN
It confirms the engine and lists the campaigns for that model year. Whether a recall is still open on this VIN: nhtsa.gov/recalls.

2Cold start and idle
Ask the seller not to start or warm the engine before you arrive. Most of these only show on a cold engine; then let it idle and listen.
- Common-rail injector wear / nozzle carbon (03-07)2003-2007
- cold-start rattle and white smoke that clears warm; uneven idle; rising fuel consumption
- Fuel lift (transfer) pump failures1998-2004
- hard starts and long cranking; power fade under load; injector pump noise on the 24v
- VP44 injection pump death (24v ISB)1998-2002
- dying pump symptoms: hard starts, dead pedal above ~2000 rpm, surging, then no-start
- Intake grid heater / relay failures (cold starts)1998-2007
- long crank and white smoke in cold weather; 'wait to start' behaviour that ignores temperature
3Scan for codes
A basic OBD-II reader shows stored and pending codes; on a diesel ask a shop for a full scan if you can. These are the codes our records tie to known failures on this engine.
| Codes | Points to |
|---|---|
| P7100 | 12v overflow valve weak (hard start / low power) |
| P0216, P1688, P1689 | VP44 injection pump death (24v ISB) |
What a code means on each engine: trouble codes by engine.
4Hood up and underneath
Engine off, in daylight, with a flashlight.
- 53-block casting crack (24v ISB)1999-2002
- coolant loss with no external leak traces; crusty white/green residue high on the block, under the intake side near the injection pump
To confirm: locate the '53' casting number on the block side (engraved, ~2 cm) and inspect the pump-boss area for weeping/crusting
5Test drive
At least 20 minutes, including a highway run and a hill or a load if you can.
- Common-rail injector wear / nozzle carbon (03-07)2003-2007
- metal-on-metal tick from a failing injector; copper crush washer blow-by at the sleeve
- Killer Dowel Pin (KDP) on the 12v P-pump engines1994-1998
- no warning until the pin falls: grinding/clatter from the front gear housing, then sudden engine stop or damage
- Turbo wastegate actuator seize / boost control (HX35W, HE351CW)
- overboost spikes and defuel (actuator seized closed); or lazy spool and low boost (stuck open)
6After the drive
Hot engine, parked: the dipstick and the coolant reservoir tell you what the drive did. Then shut it off and restart it hot: some failures only show on a hot restart.
- 12v overflow valve weak (hard start / low power)1994-1998
- long crank when warm, low power and smoke under load, erratic high-idle
To confirm: gauge the P7100 fuel pressure (should hold spec at cranking and idle)
- Head gasket failure on tuned common-rail trucks (03-07)2003-2007
- coolant push-out under boost (degas overflow), combustion bubbles in the degas bottle
To confirm: combustion-leak test at the degas neck + boost-event logging (overboost spikes)
What it costs if you find it
Parts budgets from our repair recipes for this engine: Dave's Auto Center's prices where Dave's lists the part, approximate OEM and maker catalog prices (rounded) for the rest; labor not included. A campaign listed next to a failure may make the repair free on a covered truck: check it by VIN.
| Failure | Years | Repair budget | May be covered by |
|---|---|---|---|
| Common-rail injector wear / nozzle carbon (03-07) | 2003-2007 | about $2,600-$2,800 in parts, plus parts we have no price for | - |
| Fuel lift (transfer) pump failures | 1998-2004 | about $120-$1,950 in parts | - |
| Killer Dowel Pin (KDP) on the 12v P-pump engines | 1994-1998 | about $220-$520 in parts | - |
| VP44 injection pump death (24v ISB) | 1998-2002 | about $2,100 in parts | - |

Built from the failure-mode records, recalls registry and repair recipes on this site; every line links to its source page with tiers and sources. This is a list of documented failures to check, not an inspection: a pre-purchase inspection by a diesel shop sees what we cannot. Years show where a failure covers only part of the engine's run.
