7.3 Power Stroke (1994.5-2003): misfire, rough idle, hesitation or surging

4 documented failures on this engine list this among their signs. They are ordered from the most widespread down. Each one shows the signs in its record's own words, how to tell it from the others, the usual fix and any recall or bulletin.

  1. CPS (camshaft position sensor) failure Very common · 130 complaints · 2 recall/bulletins
  2. Under-valve-cover harness (UVCH) failure Common · 1 complaints · 1 recall/bulletin
  3. IPR valve failure / o-ring degradation Common · 1 recall/bulletin
  4. IDM (injector driver module) failure Occasional · 1 complaints

Not sure which engine you have, or want the recalls for your exact truck? Check your VIN: it names the engine and lists the campaigns for that model year. Whether a recall is still open on your VIN: nhtsa.gov/recalls.

verified How widespread: Very common 130 NHTSA owner complaints match it, out of 667 checked

How it shows up

  • intermittent stumble then recovery; no warning
Its other signs
  • engine dies while driving / no-start, often hot; restarts after cooling
  • tach does not move while cranking (the signature tell)
  • repeat failures with aftermarket sensors are common

How to tell it apart

  • crank and watch the tach: no movement = CPS (the roadside diagnostic)
  • check for RPM signal with a scan tool; DTC P0340-family when it sets
  • replace with an OEM-quality sensor - cheap aftermarket CPS units are notorious for repeat failure and rough running

Usual fix

replace the CPS (OEM/OEM-quality only; carry a spare in the glovebox - community standard practice)

How it shows up

  • rough running / dead hole(s); misfire that comes and goes
Its other signs
  • glow plugs reading dead on a scanner (harness, not plugs)
  • melted/burnt connector at the valve cover (9/16' run through the gasket)

How to tell it apart

  • scan glow-plug circuit resistance per bank: open circuit = harness or plug, not GPR
  • inspect the exterior connectors at the valve covers for melt/looseness
  • wiggle-test harnesses while running to reproduce a dropout

Usual fix

replace the UVCH / valve-cover gasket-integrated harness (correct 2-connector OBS or 1-connector 99-03 design)

aggregated How widespread: Common

How it shows up

  • no-start or rough hot running; IPR duty pegged high
Its other signs
  • ICP erratic vs commanded
  • IPR screen found clogged with debris (pump wear or oil breakdown)

How to tell it apart

  • scan IPR duty vs ICP actual (high duty, low pressure = IPR circuit or pump)
  • inspect the IPR screen when accessible - debris tells you the pump is shedding
  • replace o-rings / valve and retest before condemning the HPOP

Usual fix

IPR o-ring kit / replacement IPR valve (fit new screen)

verified How widespread: Occasional 1 NHTSA owner complaints match it, out of 667 checked

How it shows up

  • dead/multiple dead holes or no-start with good oil pressure data
Its other signs
  • moisture corrosion found at the IDM connector (fender-mounted box)
  • DTCs pointing at injector circuit(s)

How to tell it apart

  • injector-circuit DTCs + good ICP/IRP data points IDM-side
  • swap-test or inspect the IDM connector for corrosion (under-hood fender location takes water)
  • check wiring before replacing the module

Usual fix

replace the IDM (rebuilt units common; verify the variant fits the era)

Order: how widespread each failure is in its record (near-universal, very common, common, occasional, rare), then the number of NHTSA owner complaints that match its keywords. Complaint counts are keyword matches, not a manual review of each complaint. A symptom listed here means the failure is documented with that sign; it is not a diagnosis of your truck.
