Symptoms
- a loud diesel-like rattle on cold start that fades as oil pressure builds
- rattle at hot idle, occasionally with a rough first few seconds
- timing/cam DTCs (P0011/P0012 family) with the rattle
- check-engine light with cam timing codes
Root cause
The 5.4L 3V phaser (variable cam timing actuator) wears internally - the locking pin and vane clearances open up and the loose phaser rattles before oil pressure locks it quiet. Oil discipline (viscosity, intervals) is the prevention; the fix is phaser replacement with the timing chain front end open. The 6.8L 3V V10 is non-VCT and is not the rattle source. Ford's design-change bulletin TSB 04-24-24 (CAMSHAFT AND LASH ADJUSTER DESIGN CHANGE ON A 5.4L 3V ENGINE, MY2004-2005) is the OEM anchor - surfaced by the wide F-150/Expedition mining pass.
Diagnosis
- confirm the noise is at cold start and fades with oil pressure (phaser signature)
- scan for cam-timing DTCs; check VCT solenoid operation
- oil condition first - sludge both causes and mimics phaser noise
Fixes
replace the phasers (in pairs) with the timing chain kit while the front end is open; check VCT solenoids (phaser replacement) - the definitive fix; front-of-engine labor
